Choose from $500, $1,000, $2,500, $5,000 or $10,000. This is how much you will pay in covered services before your insurance benefits kick in.

Next, pay a percentage of covered services.
After your deductible is met, you pay 20% of the next $5,000 of covered expenses. This is your co-insurance.

Relax and Recover.
Thereafter, your plan pays 100% of remaining covered expenses up to the plan maximum of $1 million for each covered person.
